How often are the brakes of a Formula 1 car changed?

How often are the brakes of the car changed? Is the race always started with a fresh set of brakes?

During a weekend the teams usually use one or two sets of brakes. Friday and Saturday practises are often driven with a dedicated set of brakes. Sometimes a new set is changed on Saturday morning, so it can be verified during the practise session.
